To use a template when creating a new form, click the Templates tab on the Forms Home Page  > . Choose Forms by Category or Templates > Forms by Tag from ' main menu.

Forms by Tag which display along with Controls by Category and Controls by Tag.

You’ll see the categories or tags you created when you published your templates.  Some tags are in larger font; this means you’ve used these tags for more than one template. Click any category or tag to display a list of matching templates.

  • Test the template by clicking (the Try icon)— the template displays as a fully functional form so you can enter and validate data before deciding whether you want to use it.
  • Use the template in a new form by clicking (the Add icon).  By definition since this is a template you will be able to use it in as many forms as you want.  Once you’ve used it to create your form, you can customize your form without ever affecting the template.Email the template by clicking (the EMail icon). This launches a new message in your default email system. The email message includes a link to the template, which the recipient can view and test.

    

If you choose to add the template, you’ll be walked through a wizard that allows you to make minor modifications before your new form is launched. If you like your template exactly as it is, skip the wizard by clicking Finish. This is the same wizard you see when you create a new form without using a template.

Form templates can also be added to the new form wizard which appears every time you create a new form by clicking icon in the forms home page Forms Home Page.

ONLY forms published by the tenant admin get added to this wizard. Forms published by non-admins are only added to the Examples area. To add a template to the new form wizard:
